I have found the solution. It was, in fact, a r/w rights error. I have added following users to my profile folder: IUSR Local Service Network IIS_IUSRS Service I know I went overkill here but I wanted to be sure without giving everyone access rights. I will disable them one by one and find the right one(s) needed to properly run SQL Server Express 2012.

If you want to store binary files in the database, you can serve them directly without writing them to a filesystem first. All you have to do is write a PHP script (or whatever) to send the data in response to an incoming HTTP request, with the proper media type in the response HTTP header. So if we're talking about images, in your HTML you'd do something like this: <img src="/your_img_script.php?image=1234"></img> Then just write your_img_script.php to lookup image ID 1234 and dump the data out with a image/jpeg (or whatever) content type header.

Express request routes twice on browser page load
This was confusing as all over the internet this issue is solved by rectifying the favicon thing. But this was stupid jquery $.get() call which was firing blank (it was calling the same url as the page url) when the variable was not present. I wrapped the ajax call and problem solved.
